Treatment of Helicobacter pylori

Bismuth/Tetra/Metro "Triple Therapy" (with acid reduction)
cure rate 95% in 7-14 days
    • Famotidine 40 mg PO qd
      (or an equivalent dose of an alternative H2 blocker) or Omeprazole 20 mg PO qd (or an equivalent dose of an alternative PPI)
    • Tetracycline 500 mg PO qid
    • Metronidazole 250 mg PO qid
    • Bismuth subsalicylate 2 tabs PO qid

Omeprazole, amoxicillin, metronidazole
cure rate 90% in 7 days
    • Omeprazole 20 mg bid
    • Amoxicillin 500 mg qid
    • Metronidazole 250 mg qid

Omeprazole, clarithromycin-metronidazole
cure rate 90-95% in 7 days
    • Omeprazole 20 mg bid (or 40 mg qd)
    • Clarithromycin 500 mg bid
    • Metronidazole 500 mg bid

Omeprazole, amoxicillin, clarithromycin
90-95% cure in 14 days.
    • Omeprazole 20 mg bid
    • Amoxicillin 500 mg qid
    • Clarithromycin 500 mg bid
This heavy-duty regimen should only be used if the patient has already failed on other therapies, and only after sensitivities have been determined.
