Calcium Carbonate (CaCO3)

 

Actions and Uses

    Antacid and Phosphate Binder. Binds dietary phosphate and neutralizes gastric acid. It is used as a phosphate binder (to reduce phosphate level in chronic renal failure pts), an antacids, and as a calcium supplement.

 

Available As

    Tablets (500 or 650 mg), oral suspension (1250 mg/5mL)

 

Dose and Administration

  • Dosage ranges from 650 mg once daily to 1950 mg three times daily.
  • 40% of CaCO3 wt is elemental Ca++
  • Avoid giving within 3 hrs of oral mycophenolate, oral fluoroquinolones (ofloxacin, cipro, etc) and other interacting drugs.

 

Pharmacokinetics

  • Ca++ absorption is regulated via parathormone (PTH) and calcitriol.

 

Side Effects

  • Constipation
  • Milk-alkali syndrome (metabolic alkalosis)

 

Drug Interactions

  • Interferes with the absorption of many drugs.
  • Avoid administring divalent cations like calcium within 2-4 hrs of mycophenolate, oral fluoroquinolone, and other interacting medications
